Introduction

Takeda Pharmaceutical Company Limited recognizes and respects the privacy rights of individuals with regards to their personal data. The myPNtracker Privacy Notice (“Notice”) explains what type of personal data we may collect from you and how it will be used when you are using the myPNtracker Web Portal. For general viewers of this website, along with more detailed information on how Takeda processes your personal data, please refer to Takeda’s Privacy Notice at https://www.takeda.com/privacy-notice/.

If you (“User” or “You” or “Your”) are a healthcare professional (“HCP”) who creates a registered account on the myPNtracker Web Portal the following additional privacy information applies to the use of Your account.

myPNtracker Web Portal is made available as a website (“Web Portal”) provided to You by Takeda (for more details – see How to Contact Us section below).
Before using the Web Portal, You should read this entire Notice (together with the Terms of Use). At the end of this Notice we will ask You to acknowledge, and to provide Your consent to, the processing of Your personal data as described. If the processing of personal data as described in this Notice is not acceptable to You, do not use and/or submit any information to this Web Portal.

The myPNtracker Mobile Application, used to share data with healthcare professionals who are registered on this Web Portal, is not intended for users who have not reached the age of majority in the province or territory where they live without consent from their parents or guardian.

If You have any questions concerning Takeda’s privacy practices or wish to access or correct personal data that Takeda has collected from You, please contact us as described in the “How to Contact Us” section below.

What the Web Portal can be used for

The Web Portal allows You to view the Personal Patient Information of any patients who have agreed to share their data with You. Patients receiving parenteral nutrition can enter the following Personal Patient Information on their mobile application:
• weight;
• fluid intake and output;
• injection sites;
• appointments;
• medications;
• treatment response.

Your patients can use the myPNtracker Mobile Application to share the information with You through the myPNtracker Web Portal or by providing You with reports based on the information they have entered into their myPNtracker Mobile Application. myPNtracker Web Portal is intended to be used strictly as a digital diary and you are under no obligation to monitor or respond to the disclosed information. The use of the app is not intended to replace the treating physician, pharmacist or other qualified healthcare professional that is responsible for managing the patient’s condition on any treatment strategy based on the data collected and disclosed.

What information is collected

When You use the Web Portal, Takeda will collect data from You, including information You have shared and which directly identifies You (e.g. name, email, etc.) or general information which indirectly identifies You (e.g. frequency of visits, pages viewed, etc.). Your information may be kept in an identifiable format, or in aggregate format which means that You cannot reasonably be identified from it. The information we collect about You (collectively and individually, the “HCP Information”) includes:
• Language;
• Country;

To set up a User Account, to see the personal patient information Your patient has agreed to share with You, the following HCP Information about You must be entered in the Web Portal.

HCP Information includes the following:
• Title;
• Name;
• Email Address;
• Password;
• Kind of Practitioner and Specialty;
• Country and Region;
• Preferred Unit of Measurement; and,
• Notification Preferences.

You are responsible for ensuring the accuracy of any HCP Information entered into the Web Portal.

Using the myPNtracker Mobile Application, patients may choose to share the following Personal Patient Information with You:
• Body data: Weight, height, mid-arm circumference, tricep skinfold, temperature;
• Intake: Oral Fluids, Oral Re-hydration Solution, Nutrition Supplement (mL), IV Fluid Hydration, IV Nutrition, Enteral Nutrition, Salt Capsules;
• Output: urine, stool, stoma and vomit; and,
• Schedule and treatment regime.

Patients are responsible for ensuring the accuracy of any information entered into the myPNtracker Mobile Application.

We will also collect HCP Information about You when You interact with us such as when You report a problem, contact or communicate with us.
We automatically collect certain data from Your device when You use the Web Portal, including data about Your language and region setting.
You are responsible for ensuring the accuracy of any HCP Information entered into the Web Portal.

How we use the information we collect

We process Your HCP Information to provide You with the above mentioned functions, as well as to manage and improve the Web Portal. We also process the collected HCP Information to provide You with optimal functionality and operation.

Keeping your information secure

You and Takeda are responsible for maintaining the confidentiality of Your HCP Information. Your HCP Information is accessible with Your email address and passcode. You should make sure Your passcode is kept confidential. You should also be aware that information displayed on Your screen may be viewed by others in Your vicinity. If at any time You have reason to believe the security of the Web Portal has been compromised please contact us immediately.
Additionally, Takeda implements commercially reasonable and appropriate technical, physical, and organizational safeguards to protect Your HCP Information from misuse or accidental, unlawful or unauthorized destruction, loss, alteration, disclosure, acquisition or access.
Service providers are required by Takeda to implement appropriate safeguards to ensure that Your HCP Information is adequately protected.

Sharing your information

A patient may choose to share their Personal Patient Information with You or others assisting with their healthcare. They do this by inviting You to register and view their data via the myPNtracker Web Portal. Once You are registered on the myPNtracker Web Portal You become listed within the myPNtracker Mobile Application and the patient must specifically choose to share their health data with You. The patient may revoke shared data privileges at any time using the myPNtracker Mobile Application. Only the patient has the ability to control who has access to their data.
Takeda may share Your HCP Information with our authorized service providers that perform certain services on our behalf such as providing storage services and providing support for the Web Portal. These service providers may have limited access to Your HCP Information, which is needed to perform their functions, but are not permitted to share or use such HCP Information for any other purposes.
We may also have to share Your HCP Information for regulatory reporting purposes or legal obligations.

Where we store your information

The HCP Information that we collect from You will be stored in encrypted format on a server. The HCP Information stored on the server may be transferred to, and stored at, a destination outside of Your home country. These destination countries may have different or less protective privacy laws than those in Your home country. The HCP Information may also be processed by any service provider appointed by us who operate outside of Your home country and their staff, and/or our own staff based outside of Your home country. By submitting or permitting us to collect Your HCP Information, You agree to this transfer, storing or processing outside Your home country. HCP Information transferred will be treated in accordance with this Notice.
Your patient may also export their Personal Patient Information into a report to share with You or others. They may share with others by printing or emailing the report. Unfortunately, the transmission of data via the Internet is not totally secure. Even though appropriate measures may have been taken to protect the content, it does not guarantee that the data is fully secure against unauthorized access or use by others. A patient choosing to email their Personal Patient Information does so at their own risk.

How long we store your information

Takeda will only retain HCP Information for as long as necessary to serve the purpose for which it was collected or to the extent necessary to comply with applicable legal requirement(s). Data will automatically be removed from the server after 1 year of inactivity on Your account.

Data Deletion

You can delete Your HCP Information from the Web Portal by clicking on the “Delete Account” button on the settings page. This will remove any, and all, data stored on the myPNtracker server, settings and information collected by this Web Portal and related to Your profile. This process cannot be undone, information is permanently deleted and You will be logged out of this account.
You can also request to have all HCP Information stored by Takeda on the Web Portal server deleted by making the request to privacyoffice@takeda.com. Takeda will delete all Your HCP Information after an inactivity of Your account for a 1 year period.

Access to and choices regarding your Information

You can change, correct or delete Your preferences and the data entered into the Web Portal at any time and those changes and/or deletions will be reflected on the Web Portal.

Acknowledge/Consent

By using this Web Portal, which is conditional upon Your pressing "I accept", You acknowledge that You are a healthcare professional, have reached the age of majority in the province or territory where You live, have read and understood this Notice, and consent to the processing of Your HCP Information for the purposes of this Web Portal, which may include the transfer of Your HCP Information outside of Your home country.
